IMPORTANT UPDATE
From Monday 4th December we will no longer be accepting prescription requests from Pharmacies on behalf of patients. You must request prescriptions yourself via the surgery. This is to improve patient safety and to enable us to keep accurate patient records.
Prescriptions can be requested in the following ways:
· The NHS app
· A SystmOne account (this can be set up on request by contacting reception)
· Using a repeat slip or piece of paper with medications listed and handed to the surgery reception or put in the letter box outside the surgery (this can be done by a friend or family member on your behalf)
· The surgery prescriptions email address: hds.prescriptions@nhs.net

Repeat Prescriptions
Repeat prescriptions may be ordered using the tear-off computer printout from your last prescription
We ask that you leave sufficient time for your repeat prescription to be issued before your current supply of medication runs out. Please also remember to plan ahead when you are going away on holiday or for over a Bank Holiday.
Please remember to tick all the items which you require on the slip attached to your last prescription.
Please note that prescription requests are NOT taken over the telephone.
Please allow 2 working days excluding weekends and Bank Holidays so that prescriptions can be prepared and signed by the doctor.
