How To Register

We are using a new online service called [Register with a GP surgery] that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Out of Area Patient Information

Out of Area online registration is currently unavailable due to a technical issue. The expected date for this to be corrected is 23/11/24. If you wish to regsiter as 'out of area' before then, please collect a paper registration form from Reception. We apologise for any inconvenience

The current catchment areas for Hawthorn Drive Surgery are those living within the IP2 and IP8 post codes. In the past this meant the surgery only accepted patients from those post codes. However the surgery has taken the decision to open these boundaries.

 

We have opened up registrations for 'out of area' patients to accept both;

  • those from outside the catchment area of IP2 and IP8 post codes
  • those who previously lived in the IP2 and IP8 post codes but have moved out of the catchment area.
 

Under the Out of Area Registration Guidelines, if you are an 'out of area' patient you will not be entitled to Home Visits. Please bear this in mind before continuing with your registration. Should you find yourself in need of a Home Visit please contact NHS 111, who will be able to provide this service to you.

Other than Home Visits 'out of area' patients are entitled to the same medical care from our practice as any other patient.

 

New Patient Check

All new patients are invited to attend for a “New Patient Check”. This check is very important as it can sometimes take a while before we receive your full medical records. You will be given a questionnaire and a urine specimen container.

Please bring a repeat medication list with you, if you have one, a urine sample and your completed questionnaire, when you attend your appointment.

 

Non English Speakers
